Dear Parents,

Please be informed that in accordance with the Ukrainian Law and a letter of your obligation submitted to the State Department on Adoption of Ukraine, the Consular Office of the Embassy of Ukraine to the USA should include your adopted children into the list of Ukrainian citizens permanently residing in the USA.

In order to process appropriate paperwork we kindly ask you to mail to the Consular Office completed registration form with one picture of your child,  a photocopy of the original Ukrainian court decree on adoption and your child’s ORIGINAL Ukrainian Passport.

The original passport is required to put a stamp and the official seal into it to complete the registration.

Please include a return self-addressed postage-paid envelope to return the child’s passport to your address.

Registration papers are requested to be mailed  to the Consular Office of the Embassy of Ukraine at: 3350 M Street NW Washington, DC 20007.

In case you are going to change your place of residence, you are kindly requested to inform the Consular Office on your whereabouts.
